Diabetes Nephropthy

I am type 2 diabetic. Juvenille. I have developed diabetic nephropathy since last 6 months. My ceratinine levels is 4.0-4.2. Also my blood pressure is in range of 160/90

Please guide me as to how to reduce my ceratinine levels. Doctors have asked me to be prepared for dialysis or transplant.
