Shrinking kidney

I 40 yr female, diagnosed with both kidneys shrinking. Total loss of function on right kidney left kidney functioning only, also shrinking. On medication for high blood pressure, anemia(iron), gout & low red blood count. Severe joint pains. my GFR is 18. Please advise when dialysis or transplant will be happen or will i recover with 18% of one kidney

As your kidneys are showing shrinking with related symptoms and you are on medications for various ailments and even you are suffering from gout and joint pains which are interrelated.
